GP Referral Guidelines

Step 1: Read our referral guidelines

The Sussex Community Dermatology Service is a secondary care Dermatology service for patients registered with West Sussex Primary Care Trust. Referrals can be made by any NHS healthcare professional but must adhere to the referral guidelines. Please download the referral guidelines below to ensure that the patient can be seen within the service.

Common Exclusions: 
Leg Ulcers
2-week rules
Botox for Hyperhidrosis
Cosmetic skin lesions or problems
Patients requiring transport - As we are a community service we are unfortunately unable to provide a transport service for patients. All clinics do however have full disabled facilities and access.


Step 2: Complete the referral form or letter

We have prepared a standard referral template for the service to make it easier for you to refer patients on to us with all the information we need. Simply fill in the patient’s details and choose which locations they would like to be seen at. Where possible we will always try to book patients in at their chosen location or close to their home.

“The SCDS offers a fast and efficient referral service for our patients here at The Lime Tree Surgery with dermatological conditions. We access the service on behalf of patients via the Choose & Book website and have found it to be a popular option for patients wishing to avoid the longer waiting times at WASH. The staff at the SCDS have always been extremely helpful and courteous with our enquiries and go a long way to ensuring that the patient's pathway is as smooth and hitch free as possible. They always inform us if there is a shorter wait at Hove, rather than Steyning which is useful and helps patients. They always return our phone calls promptly and offer good and thorough advice. Patients who have their own transport or are willing to travel slightly further afield have chosen this referral service over the local hospital, although some of our older patients still prefer to wait to see the dermatologist at the hospital, especially if they require transport or have difficulties with mobility. All in all, a first class service and an asset to health services locally.”
