Information for GP’s
We have prepared a standard referral template for all services to make it easier for you to refer patients on to us with all the information we need. Simply choose the form that is relevant to your CCG, fill in the patient’s details and choose which locations they would like to be seen at. We will always try to book patients in at their chosen location or close to their home however in some cases this may not be possible. All patients are offered an appointment within six weeks of the referral being received.

Sussex Community Dermatology Service (SCDS) encourages patients to complete a patient satisfaction survey following their appointment at one of our many clinic locations across Sussex, Surrey and Kent. Results are reviewed monthly to identify opportunities for improvement of our services and are collated annually to conduct a patient satisfaction audit.
